Medical Authorization

I certify that the participant is physically able to participate in clinic activities. In the event of an emergency, I authorize medical treatment deemed necessary by qualified medical personnel. I understand that I am responsible for any medical expenses incurred.

Assumption of Risk and Release of Liability

I understand that participation in baseball and softball activities involves inherent risks, including but not limited to serious bodily injury, illness, permanent disability, paralysis, property damage, and death. I voluntarily assume all risks associated with participation in the Minority Prospects Free Youth Baseball & Softball Clinic. I release, waive, discharge, and covenant not to sue Minority Prospects, its officers, directors, employees, volunteers, sponsors, partners, facility operators, instructors, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of participation in the clinic.

Photo, Video & Media Release

I grant Minority Prospects the irrevocable right to photograph, record, livestream, and use the participant's name, image, voice, likeness, and biographical information in promotional materials, social media, websites, broadcasts, Minority Prospects TV, marketing materials, and future event promotions without compensation.

Communicable Disease Acknowledgement

I understand that participation in public events may expose participants to communicable diseases. I voluntarily assume all risks associated with such exposure and release Minority Prospects from related claims.
